Please note that June 27th, 2011 is the Deadline (without a late fee) for the following requests:

1. Petition for Graduation for the fall 2011 Trimester (graduation date 12/19/2011).
2. Change program/major effective in fall 2011 trimester.
3. *Change graduation requirements to the current catalog requirements (2011) effective in fall 2011 trimester.

* Applies to those students whose study plan template is before 2011 and wish to change program or catalog.

Submit the above requests online at http://npuosc.npu.edu by filling out the following forms:

• Form Number SR-10: Petition for Graduation
• Form Number SR-5: Change Program/Major
• Form Number SR-3: Change Catalog
